Thank you for scheduling an appointment at the Center for Spine & Orthopedics. Our physicians are experts in spine and orthopedic surgery, non-surgical treatment and pain management. When meeting with our physicians, they will diagnose your condition and take time with you to discuss treatment options for the best possible outcome. We are excited you have selected the Center for Spine & Orthopedics as your spine and orthopedic specialists, and we look forward to serving you. The Center for Spine & Orthopedics requires that you bring all of the following items to your initial appointment so that we can best treat your condition or injury.

  • Completed patient forms

  • Valid form of identification

  • List of all your medications

  • Insurance card

  • Payment (cash, check, Visa, Mastercard, Discover or American Express)

  • Phone and fax numbers for your referring doctors and primary care physician

  • MRI or x-ray films and all other records related to your injury or condition

To save you time, extra travel and more waiting for your convenience, we offer on-site access to an array of ancillary (diagnostic/ therapeutic) services, to name a few:

Your x-rays are ordered and can be accomplished before or immediately after your examination to get a clear picture of your source of pain or potential disorder. Our experienced radiology technicians can perform them on-site in CSO’s main facility. Results are available almost immediately, and your physician will discuss your diagnosis and treatment options during your initial appointment.

Your physician may need an MRI or other high-resolution diagnostic modality before or after your visit to gain a clearer picture of your condition. You can schedule an MRI or other diagnostic modality at Health Images. On the same visit, you can also schedule a follow up visit with your CSO provider. For added convenience, simply pick up MRI results just before your next follow-up appointment. For details, visit healthimages.com.

To save you another drive and possibly a long wait at another facility, CSO offers on-site urine testing, which may be required for drug screening on the day of your appointment.

To save another drive to another site, CSO offers durable medical equipment (DME), such as orthopedic boots, braces and foot inserts, that you can pick up in our office should your physician order it. Our goal is to save time and save you from waiting.

CSO provides on-site electro-diagnostic treatment, such as EMGs. An EMG analyzes the electrical activity in your muscles so your physician can learn more about nerve functioning in the arms and legs. Small, thin needles are placed in the muscle to record the electrical activity. Your doctor will listen and watch a TV-like screen that broadcasts the electrical signals. The North Metro Surgery Center next door is a fully-licensed ambulatory surgical center for orthopedic, spine, podiatry and outpatient procedures. Patients enjoy a warm, comfortable environment, enabling them to return home the day of their procedure or stay several nights for observation. Patients also receive individualized attention to minimize the stress of surgery. A highly skilled team of registered nurses and medical technicians will assist your surgeon. About MRI

Magnetic Resonance Imaging (MRI) uses a magnetic field and pulses of radio wave energy to create images of organs and structures in the body. MRI can often provide different information about structures than can be seen with an x-ray.

MRI’s can often provide different information than other imaging tests.

ultrasound or computed tomography (CT) scan; it can also show problems that cannot be seen with other imaging tests.

MRI is used to diagnose bone and joint problems, such as arthritis, fractures, bone tumors, bone marrow problems, torn ligaments or tendons. It can also be used to check spinal discs and nerves for various conditions, including stenosis, bulging discs and spinal tumors.
